We are in-network with Aetna, Anthem BCBS, Cigna, UnitedHealthCare, Oxford, and Oscar insurances. If you are out-of-network, we can provide you with Superbillls. Self-pay rates are $290 for your first appointment and $150 for standard follow up appointments.

Complete Our New Patient Packet
Complete our standard questionnaire and forms to establish care. If you are using insurance, you will be assigned either a Headway or Alma account depending on your insurance. This is utilized only as a billing service so once your Headway or Alma account is completed, you do not need to attend to it unless you have questions about billing or insurance. Lastly, activate your patient portal. We are a technology forward clinic and heavily use our patient portal to send back and forth information!

As we are a virtual clinic, all our appointments are conducted via private Google Meet links. Our Patient Portal also offers convenient ways to manage your care during and after office hours.
